<h3><font style="color: rgb(0, 0, 0);">患者王某，男性，22歲，漢族。主因間斷頭疼頭暈伴右側(cè)面部感覺麻木1月入院。<br>入院查體：神智清楚，右側(cè)顏面部感覺明顯減退，余神經(jīng)系統(tǒng)查體未見異常。</font></h3> <h3><font style="color: rgb(0, 0, 0);">以上為頭顱CT檢查結(jié)果，患者右側(cè)中、后顱凹可見巨大腫物占位影。</font></h3> <h3><font style="color: rgb(0, 0, 0);">矢狀位頭顱CT示：鞍旁及橋小腦角區(qū)占位</font></h3> <h3><font style="color: rgb(0, 0, 0);">增強(qiáng)MRI平掃及增強(qiáng)顯示中、后顱凹，即鞍旁及橋小腦角區(qū)腫瘤。</font></h3> <h3><font style="color: rgb(0, 0, 0);">頭顱CTA顯示腫瘤與周圍血管比鄰情況。</font></h3> <h3><font style="color: rgb(0, 0, 0);">根據(jù)患者臨床表現(xiàn)及影像學(xué)檢查，入院診斷：三叉神經(jīng)鞘瘤</font></h3> <h3><font style="color: rgb(0, 0, 0);">三叉神經(jīng)鞘瘤（trigeminal nerve schwannoma）發(fā)生率僅次于聽神經(jīng)瘤，是第二位的顱內(nèi)神經(jīng)鞘瘤，三叉神經(jīng)鞘瘤約50%位于顱中窩；30%位于顱后窩；20%為啞鈴型，同時(shí)累及顱中、后窩。<br>啞鈴型三叉神經(jīng)鞘瘤會沿著三叉神經(jīng)走行，由腦干腹側(cè)面發(fā)出，向上前側(cè)方經(jīng)腦橋小腦角池走向巖頂部。在三叉神經(jīng)孔處穿過顱中窩的硬膜，位于小腦幕側(cè)聯(lián)合和巖上竇的下方。小腦后動脈和小腦上動脈跨過其根部上方，小腦前動脈自其下方經(jīng)過。巖靜脈位于神經(jīng)根的側(cè)方和后方，在巖上竇下方進(jìn)入三叉神經(jīng)孔?；嚿窠?jīng)位于腫瘤的上方，第Ⅶ、Ⅷ對腦神經(jīng)位于腫瘤下方。</font></h3> <h3><font style="color: rgb(0, 0, 0);">完善各項(xiàng)術(shù)前檢查以后，于全麻下行三叉神經(jīng)鞘瘤切除術(shù)，患者取側(cè)臥位，三釘頭架固定穩(wěn)妥。</font></h3> <h3><font style="color: rgb(0, 0, 0);">切口位置及形態(tài)</font></h3> <h3><font style="color: rgb(0, 0, 0);">骨瓣下緣與顴弓平齊，弧形剪開硬膜，向上翻起。（此為術(shù)后補(bǔ)照硬膜切開示意圖）</font></h3> <h3><font style="color: rgb(0, 0, 0);">小心抬起顳葉，釋放腦脊液后，發(fā)現(xiàn)此入路腫瘤近在咫尺</font></h3> <h3><font style="color: rgb(0, 0, 0);">腫瘤將小腦幕向上頂出，突出明顯</font></h3> <h3><font style="color: rgb(0, 0, 0);">十字切開腫瘤包膜，先行瘤內(nèi)減壓。</font></h3> <h3><font style="color: rgb(0, 0, 0);">前部腫瘤徹底切除</font></h3> <h3><font style="color: rgb(0, 0, 0);">向后繼續(xù)切開小腦幕，繼續(xù)瘤內(nèi)減壓</font></h3> <h3><font style="color: rgb(0, 0, 0);">可以看到后方的小腦幕緣，及滑車神經(jīng)。</font></h3> <h3><font style="color: rgb(0, 0, 0);">切開小腦幕，進(jìn)入后顱凹</font></h3> <h3><font style="color: rgb(0, 0, 0);">發(fā)現(xiàn)后顱凹腫瘤體積巨大，張力過高，繼續(xù)內(nèi)減壓</font></h3> <h3><font style="color: rgb(0, 0, 0);">減壓充分后將腫瘤包膜向上翻起</font></h3> <h3><font style="color: rgb(0, 0, 0);">可見橋腦腹側(cè)出腦干位置殘留的三叉神經(jīng)起始部</font></h3> <h3><font style="color: rgb(0, 0, 0);">基本大部切除腫瘤后情況</font></h3> <h3><font style="color: rgb(0, 0, 0);">發(fā)現(xiàn)巖骨下方還有少部分腫瘤殘留，有一定的死角</font></h3> <h3><font style="color: rgb(0, 0, 0);">為了徹底的切除腫瘤，高速磨鉆磨除巖骨尖部。</font></h3> <h3><font style="color: rgb(0, 0, 0);">徹底切除后顱凹腫瘤</font></h3> <h3><font style="color: rgb(0, 0, 0);">止血滿意后，鋪填止血材料</font></h3> <h3><font style="color: rgb(0, 0, 0);">縫合手術(shù)切口</font></h3> <h3><font style="color: rgb(0, 0, 0);">術(shù)后第二天，患者意識清楚，恢復(fù)良好，無嚴(yán)重并發(fā)癥。</font></h3> <h3><font style="color: rgb(0, 0, 0);">術(shù)后第二天復(fù)查CT情況，腫瘤切除滿意</font></h3> <h3><font style="color: rgb(0, 0, 0);">最后要感謝為此次手術(shù)提出過寶貴意見和建議的西京醫(yī)院科室的各位老師，還有我的手術(shù)團(tuán)隊(duì)的同志們。</font></h3> <h3>患者術(shù)后一年復(fù)查的效果</h3>
